This paper is not available on-line. The copies were lost in 1994 during a relocation of residence. Because the paper was not published, all that remains is the paper title (that had been on the author's curriculum vitae) and the author's recollections.

The paper was a review of the topic at that time, and was written as part of the medical school curriculum. As such, the topics covered can be found in most text books or literature searches, the information of which is surely more comprehensive now than in the early 1980's when the paper was written.
